Since USA laws only allow you to gift 10 thousand dollars a year give her that or pay some medical bills directly. Your sister can also ask the hospital what charity programs they have for the bills. I would make sure that only the medical bills are being paid so that you honor your Dad's wishes but take care of the young boy. I would not give her the funds
that Dad did not want her to have as she can then spend how she wants.
